Authorization & Waiver of Responsibility

I, the undersigned parent or legal guardian, give permission for my child to participate in the Community Bible Church Kids Fun Zone activities, including but not limited to inflatable attractions, games, sports activities, obstacle courses, face painting, and other supervised recreational activities.

I understand that participation in these activities involves inherent risks, including but not limited to slips, falls, collisions, sprains, bruises, broken bones, and other injuries that may occur despite reasonable supervision and safety precautions.

I certify that my child is physically able to participate in these activities and that I have disclosed any medical conditions that could affect participation.

In consideration of my child's participation, I voluntarily assume all risks associated with these activities. To the fullest extent permitted by law, I release and hold harmless Community Bible Church, its pastors, employees, volunteers, and event workers from any claims, demands, causes of action, damages, or liability arising out of or related to my child's participation, except in cases of gross negligence or willful misconduct where prohibited by law.

I authorize Community Bible Church volunteers or emergency medical personnel to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.

*Please list all children and their ages who will be participating in the Fun Zone Event.

*Parents parent/guardians must be present the entire time for any children 12 and under.

*Any children under  18 years of age participating must have a parent sign the waiver.
